Registering with Kingthorne Practice

We currently have an OPEN list, which means that we are accepting new patients from the following postcode areas:

DN1 1, DN1 2
DN2 4, DN2 5, DN2 6
DN3 1, DN3 2, DN3 3
DN4 5, DN4 6, DN4 7

Unfortunately, we are unable to take new registrations from outside these areas.

If you wish to register please ask for a Registration Pack from Reception at either surgery, or print off and complete the GSM1 form below. Once you have completed the GSM1 form, return it to the practice and ask to arrange a New Patient Health Check. Once you have been for your New Patient Health Check you will then be registered with the practice.

New Patient Health Check (NPHC)

You will need to attend for this in-order to complete your registration at the practice.
The NPHC involves checking:

Height
Weight
Smoking status
Alochol intake
Ethnicity
Diet
Exercise
Family History of medical problems
Current medication
Allergies
Health problems
Urinalysis

Please bring a urine sample with you and all the medication that you are taking.
