Prescriptions – How to order
Fill in a prescription request form or underline the items needed on the tear-off side of your prescription and drop it into the surgery.
If a stamped addressed envelope is supplied your prescription can be returned via Royal Mail.

Long term conditions
If you suffer from a long term condition you will be invited for an annual check in your month of birth. This is an opportunity to check that your condition is being well managed, allows you to ask questions about your condition and change treatments if appropriate. The review could be just a questionnaire or a Face to Face appointment with the Health Care Assistant. You will be advised by text message or phone call when your check is due around your birthday.
If you are having any problems with your condition, you don't need to wait for your annual review. Please contact the practice so that we can hep you manage your condition.
Long term conditions include:
- Heart disease
- Heart attacks
- Heart failure
- Chronic Kidney Disease (CKD)
- Atrial Fibrillation
- Rheumatiod arthritis
- Mental health conditions
